Within three (3) days after service of this notice, you are hereby required to perform or otherwise comply as stated herein above or quit said premises and deliver up possession of the same to the landlord/agent or the landlord/agent will institute legal proceedings for an unlawful detainer against you to recover possession of said premises, to declare said lease or rental agreement forfeited and to recover rent and punitive damages as allowed by law.

YOU ARE FURTHER NOTIFIED that by this notice the landlord/agent elects to and does declare a forfeiture of said lease or rental agreement if said breach is not remedied within the three (3) day period. The premises herein referred to are located at the following location:
